Differential Dx of appendicitis in children is Mesenteric adenitis caused by Yersinia.

To differentiate appendicitis from all other conditions, mark the point of maximum tenderness on the abdominal wall and change the position of the patient.

In all the other conditions causing pain in the Right iliac fossa the max point of tenderness will change except for Appendicitis.
